Foreign National Travel Request Form

Complete and submit the following form to request approval to pay for a foreign national's travel.

Read the following before submitting this form:

• A Banner vendor ID must be established for the traveler and must be entered in the form below. A signed W-8BEN from the foreign national is required to set them up in Banner. This form must be included with your request through LoboMart.
• Taxation staff may require more information before approving the purchase. It is possible that payment may not be allowable dependent upon the facts and circumstances.
• Approval will be for Foreign Tax purposes only and is pending receipt of required immigration documentation. This information will be outlined in the approval email.

Please attach a copy of the final approval email from Taxation, letter of invitation and your receipt in Chrome River.
